What are the advantages of the Patient’s Choice Dental Plan?
Patient’s Choice designed the plan to make dental care affordable for uninsured individuals, families and groups by providing savings of up to 50 percent on most procedures. Take advantage of the buy-one-get-one feature (available through Premium providers), and your membership will pay for itself. For every cleaning and exam you purchase through any Premium provider, you will receive the second cleaning and exam (from the same provider) for free. How much does it cost to join the Patient’s Choice Dental Plan?
A one-year membership costs $79.95 for individuals and $139.95 for families. An individual membership covers the member and all of his/her dependents under the age of 5 years. A family membership covers the member and all of his/her dependents.

How much will I save?
Discounts up to 50 percent, depending on the procedure performed and the provider you choose. Fees for particular procedures can be found on our detailed fee schedule.

What are members’ minimum and maximum ages?
There are no age limits. Dependent children are defined as 18 years or under or a full-time student up to age 23.

What will I receive when I join?
You will receive a new-member packet in five to seven business days. The packet contains a welcome letter describing how to begin using your membership, your member identification card, and a comprehensive fee schedule detailing the prices you will pay for each procedure.

When will my membership take effect?
Your membership will take effect when your payment is processed. Remember, you must present your member identification card at the time of your dental appointment to receive a discount. You will receive your member identification card within five to seven business days of purchasing your membership.

How should I begin using the Patient’s Choice Dental Plan?
As soon as you make your membership payment, you may contact any of the participating providers to schedule an appointment. Keep in mind that you will need to present your member identification card at your appointment. (You will receive it within five to seven business days of your membership purchase.) At the time of your appointment, present your ID card, and the dental office will verify your membership. The provider will charge you based on the Patient’s Choice fee schedule. You must be prepared to pay the provider at the time of service.

Do you offer an installment payment plan?
You must make one full payment when you enroll. This payment will cover your annual premium.
